Protecting your personal information is important to us. Our Privacy Policy reflects the mandatory requirements of Health Agencies under the Health Information Privacy Code 2020.

The collection and transfer of information by Telesphoros Tapui Ltd enables you to enrol with your chosen Medical Practice. The information we collect aligns with the Ministry of Health’s Enrolment Requirements for Contracted Providers and Primary Health Organisations (Enrolment Requirements). If it is not provided you may not be able to enrol in your chosen Medical Practice.

Collected Information

Telesphoros Tapui Ltd collects two types of information from you, required information and other information. Telesphoros Tapui Ltd does not store any personally-identifiable information permanently on its servers; instead all such information is transferred to your chosen Medical Practice. Your chosen Medical Practice will then pass this information on to the relevant Primary Health Organisation and the Ministry of Health.

Required Information

Telesphoros Tapui Ltd collects your full name; date of birth; place of birth and country of birth; national health index (NHI) number; gender; your enrolment eligibility and entitlement declaration; and your signature to enrolment process statements. This required information is passed on to the relevant agencies, as discussed in Recipients of the Information.

Other Information

With your consent, Telesphoros Tapui Ltd also collects other information about you. This information may include: your previous general practice; your emergency contact details; whether you have a Community Services Card or a High Health Use Card; and your chosen Medical Practice. Your chosen Medical Practice may pass this information on to the relevant Primary Health Organisation and the Ministry of Health.

Purpose

The collection and transfer of required information by Telesphoros Tapui Ltd enables you to enrol with your chosen Medical Practice, and practitioner, if selected by you.

Other Information

If you choose to provide it, the collection and transfer of your other information by Telesphoros Tapui Ltd may enable:

• your old medical records to be transferred from your old Medical Practice to your new Medical Practice;
• you to be contacted by your new Medical Practice/relevant Health Agency if required;
• Your emergency contact to be contacted by your new Medical Practice/relevant Health Agency if required; and
• your new Medical Practice to enrol you with your chosen Doctor or Nurse Practitioner.

Recipients of the Information

The intended recipients of the collected information are:

• your chosen Medical Practice;
• the relevant Primary Health Organisation that attaches to your chosen Medical Practice; and
• the Ministry of Health

Click here for the names and addresses of the individual Medical Practices and Primary Health Organisations that use our services. The only medical practice able to access the information you provide is the practice with which you choose to enrol. The Ministry of Health’s address is: PO Box 5013, Wellington, 6140, New Zealand.

Storage and security of the Information

Telesphoros Tapui Ltd does not store any personally-identifiable information permanently. All such information collected is transferred to your chosen Medical Practice. To provide and improve our service, we may collect and store non-personally identifiable information such as records of total numbers enrolled, including in sub-categories such as within age bands or by ethnicity.

To improve our service and track errors, we may temporarily log access to and use of the service, potentially including personally-identifiable information you provide. These logs will not be kept permanently, and will not be transferred, stored for longer than a defined period, or used other than to improve and maintain the service.

Your chosen Medical Practice stores the information we provide to them in line with their practice procedures and will pass it on to the relevant Primary Health Organisation and the Ministry of Health, in accordance with the Ministry of Health’s Enrolment Requirements.

No personally-identifiable information collected is permanently held by Telesphoros Tapui Ltd. To gain access to and/or correct the information collected, please contact your chosen Medical Practice, the relevant Primary Health Organisation, or the Ministry of Health.
